    Glimepiride is an oral medication used to treat type 2 diabetes. Glimepiride works by stimulating the pancreas to release more insulin. It does this by binding to sulfonylurea receptors (SUR1) on the surface of pancreatic beta cells. This binding triggers the closure of potassium channels in these cells, leading to the opening of calcium channels. The influx of calcium into the cells promotes the release of insulin into the bloodstream. The increased insulin helps lower blood sugar levels by allowing glucose to enter cells, where it is used for energy.

  • Dosage

    Glimepiride dosage recommendations are:

    • Type 2 diabetes mellitus - Initially, 1-2 mg daily, may increase in increments of 1-2 mg at intervals of 1-2 weeks according to response. Maintenance - 4 mg daily. Max - 6 mg or 8 mg daily.
  • In case of Overdose

    An overdose of glimepiride, like other sulfonylureas, can lead to severe hypoglycemia. Mild episodes of hypoglycemia can be managed with oral glucose. Severe hypoglycemic reactions are medical emergencies that require immediate intervention. In cases of severe hypoglycemia with coma, seizure, or neurological impairment, treatment with glucagon or intravenous glucose is necessary. Continuous monitoring and additional carbohydrate intake may be required, as hypoglycemia can recur even after the patient appears to have recovered clinically.

  • Missed Dose

    If you miss a dose, take it as soon as you remember. If it is close to the next dose, skip the missed dose. Do not take double doses.

  • How To Use

    Take tablet daily with food, preferably at the same time each day. Follow the dosage instructions provided by your healthcare provider.

  • When Not to Use

    Glimepiride tablets should not be used in the following situations:

    • Hypersensitivity to glimepiride or any of the product's ingredients
    • Allergic reaction to sulfonamide derivatives, as patients allergic to sulfonamide derivatives may also develop a reaction to glimepiride
  • Side Effects

    Unraveling the side effects lurking behind this product are the following:

    • Hypoglycemia
    • Temporary visual impairment (e.g., changes in accommodation, blurred vision)
    • Gastrointestinal reactions (e.g., nausea, vomiting, sensations of pressure or fullness, abdominal pain, diarrhea)
    • Hematologic reactions (e.g., thrombocytopenia, leukopenia, thrombocytopenic purpura, agranulocytosis, pancytopenia, eosinophilia, hemolytic anemia, aplastic anemia, erythrocytopenia, granulocytopenia)
    • Dermatologic reactions (e.g., pruritus, erythema, urticaria, skin eruptions)
    • Hepatic reactions (e.g., increased liver enzymes, abnormal liver function, cholestasis, hepatitis, liver failure)
    • Electrolyte disturbance (e.g., hyponatremia)
  • Precautions & Warnings

    Patients should exercise caution and consult a healthcare provider in the following scenarios:

    • G6PD deficiency or stress-related states such as fever, trauma, infection, or surgery.
    • Predisposition to hypoglycemia, including debilitated or malnourished individuals, those with inadequate caloric intake, or patients with uncompensated endocrine disorders affecting carbohydrate metabolism or counter-regulation of hypoglycemia, such as adrenal or pituitary impairment, or those who have undergone severe or prolonged exercise.
    • Mild to moderate renal and hepatic impairment.
    • Elderly individuals.
    • During pregnancy and lactation.
  • Drug Interactions

    Drug interactions with glimepiride include:

    • Insulin (may increase risk of hypoglycemia- monitor blood glucose levels closely)
    • Other antidiabetic agents (e.g., metformin) (may increase risk of hypoglycemia- adjust doses as needed)
    • CYP2C9 inhibitors (e.g., fluconazole) (may increase glimepiride levels- monitor for increased hypoglycemia)
    • CYP2C9 inducers (e.g., rifampin) (may decrease glimepiride levels- monitor for reduced efficacy)
    • Diuretics (e.g., furosemide) (may alter blood glucose levels- monitor blood glucose levels closely)
    • Corticosteroids (e.g., prednisone) (may increase blood glucose levels- monitor blood glucose levels closely)
    • Beta-blockers (e.g., propranolol) (may mask symptoms of hypoglycemia- monitor blood glucose levels closely)
    • Alcohol (may enhance or diminish hypoglycemic effects- monitor blood glucose levels closely)
    • Anticoagulants (e.g., warfarin) (may alter bleeding risk- monitor INR closely)
  • Storage/Disposal

    Glimepiride should be stored properly to maintain its effectiveness. Store the product at a temperature between 15°C and 30°C in a cool, dry place, away from direct heat, light, and moisture. Ensure the container is tightly closed to prevent contamination. For safety purposes, keep Glimepiride out of reach of children and pets to avoid accidental ingestion. It is advisable to avoid storing the product in humid or warm areas, such as bathrooms or kitchens.
