Amitriptyline is a tricyclic antidepressant (TCA). In addition to its antidepressant effects, amitriptyline has anticholinergic properties too.

Tryptanol dosage recommendations are:

• Depression - 10-25 mg daily, may increase to 100 mg daily divided into two doses, Elderly (above 65 years of age) and patients with cardiovascular disease - 10-25 mg daily, may increase to 75 mg daily, may need more frequent follow-up if doses exceed 75 mg daily
• Neuropathic pain, chronic tension type headache, and migraine prophylaxis - 10-25 mg in the evening, may increase to 25-75 mg daily, doses above 100 mg daily may require more frequent follow-up
• Bed-wetting at night - Children aged 6-10 years - 10-20 mg daily, Children aged 11 years and above - 25-50 mg daily

In case of an overdose of amitriptyline, it is crucial to contact your doctor or the nearest hospital emergency department immediately, even if you do not exhibit any symptoms. Take the medicine container with you. Overdose symptoms can include dilated pupils, rapid or irregular heartbeats, difficulties urinating, dry mouth, intestinal blockage, seizures, fever, agitation, confusion, hallucinations, uncontrollable movements, low blood pressure, weak pulse, pallor, difficulty breathing, bluish skin, decreased heart rate, drowsiness, loss of consciousness, and coma. Cardiac symptoms such as heart block, heart failure, hypotension, cardiogenic shock, metabolic acidosis, and hypokalemia are also possible. Children are particularly vulnerable to severe outcomes, including coma, cardiac issues, breathing difficulties, seizures, low blood sodium, lethargy, drowsiness, nausea, vomiting, and high blood sugar.

If you miss a dose of amitriptyline, take the next dose at the usual scheduled time. Do not take a double dose to compensate for the missed one. Setting a routine to take your medication at the same time each day can help ensure you remember to take it regularly.

This medicine can be taken with or without food. Swallow the tablets with a drink of water. Do not chew them.

Do not take Amitriptyline:

• If you are allergic to amitriptyline or any of the other ingridients in it.
• If you have recently had a heart attack (myocardial infarction).
• If you have heart problems such as disturbances in heart rhythm visible on an electrocardiogram (ECG), heart block, or coronary artery disease.
• If you are taking monoamine oxidase inhibitors (MAOIs).
• If you have taken MAOIs within the last 14 days.
• If you have taken moclobemide the day before.
• If you have severe liver disease.

Drug interactions with amitriptyline include:

• MAO inhibitors (risk of severe serotonin syndrome)
• SSRIs (increased risk of serotonin syndrome)
• St. John’s Wort (increased risk of serotonin syndrome)
• Cimetidine (increases amitriptyline levels)
• Carbamazepine (decreases amitriptyline levels)
• Phenobarbital (decreases amitriptyline levels)
• Alcohol (increased risk of sedation)
• Benzodiazepines (additive sedative effects)
• Antihypertensives (additive hypotensive effects)
• Anticholinergics (additive anticholinergic effects)

The food interactions that should be considered while using Amitriptyline Hydrochloride are:

• Grapefruit juice is known to affect the metabolism of various medications, but it doesn’t commonly interact with amitriptyline. Nonetheless, it’s always a good idea to check with your healthcare provider about specific dietary concerns.
• Consuming large amounts of high-fat foods might affect the absorption of amitriptyline. It’s usually best to take the medication with a consistent diet to ensure stable absorption.
• There’s no direct interaction between salty foods and amitriptyline, but a high-salt diet can impact blood pressure, which is worth monitoring as amitriptyline can sometimes affect blood pressure.
